Legacy Foundation of Southeast Arizona

Do you know a veteran who continues to serve their community?

Canyon Vista Medical Center and the Legacy Foundation of Southeast Arizona seek to recognize those former or current residents of Cochise County who have worn the uniform of the nation’s armed forces and have contributed to their community. Awards are based on the breadth, depth and impact of the veteran’s service to their community. The nominee must have honorably served. Nominations may be submitted posthumously.

Copies of the nomination form are available online at the Canyon Vista Medical Center www.canyonvistamedicalcenter.com/veterans-wall, and the Legacy Foundation websites or you can obtain a packets at the Legacy Foundation office, 302-01 El Camino Real, or at the concierge desk at Canyon Vista Medical Center. Nominations must be submitted by 1 AUG 2022.

Individuals selected will be honored with an individual plaque on the Veterans Wall at Canyon Vista Medial Center from November 2022 to November 2023 and honored at ceremony held at the hospital 10 November 2022 at 5:30 PM.
